  1. Two (2) executed copies of this form are required: one to be kept by the CPHA and one to be returned to the property owner.
  2. Site plans are required for ALL work submitted.
  3. The Architectural Committee meets once a month. Please plan ahead, as the approval process could take up to 30 days
